Financial Assistance

Vitality Health Collaborative offers a Sliding Fee Discount Program to ensure that individuals and families with limited financial resources have access to mental health and primary care services. Discounts are determined based on gross annual household income and household size.

Household income includes wages, self-employment income, unemployment benefits, disability, child support, and other regular income sources.

Sliding scale eligibility lasts 12 months.

Standard Service Rates

  • This sliding scale applies to individuals pursuing financial assistance for DPC services. Your monthly DPC membership fee is determined by your annual gross income:

    Individuals whose annual gross income is between $0-$10,000 would pay a monthly DPC membership fee of $10.

    Annual Gross Income : Monthly Membership Fee

    $0-$10,000: $10

    $10,001-$20,000: $25

    $20,001 – $30,000: $40

    $30,001 – $40,000: $50

    $40,001- $50,000: $65

    Individuals whose gross annual income exceeds $50,000+ would pay full price membership prices.

  • This sliding scale applies to families (3+ persons in a household) pursuing financial assistance for DPC services. Your monthly DPC membership fee is determined by your combined household annual gross income:

    Families whose annual gross income is between $0-$30,000 would pay a monthly DPC membership fee of $10.

    $0-$30,000: $20

    $30,001 – $50,000: $50

    $50,001 – $70,000: $80

    $70,001 – $85,000: $100

    $85,001 – $100,000: $130

    Families whose gross annual income exceeds $100,000+ would pay full price membership prices.

  • This sliding scale applies to individuals pursuing financial assistance for counseling services. Your fee per counseling session is determined by your annual gross income:

    Individuals whose annual gross income is between $0-$10,000 would pay a counseling session fee of $15.

    $0-$10,000: $15

    $10,001-$20,000: $30

    $20,001 – $30,000: $55

    $30,001 – $40,000: $80

    $40,001- $50,000: $100

    Individuals whose gross annual income exceeds $50,000+ would pay full price counseling fees.

  • This sliding scale applies to families (2+ persons in a household) pursuing financial assistance for counseling services. Your fee per counseling session is determined by your combined household annual gross income:

    Families whose annual gross income is between $0-$30,000 would pay a counseling session fee of $15.

    $0-$30,000: $15

    $30,001 – $50,000: $30

    $50,001 – $70,000: $55

    $70,001 – $85,000: $80

    $85,001 – $100,000: $100

    Families whose gross annual income exceeds $100,000+ would pay full price counseling fees.

Direct Primary Care Membership: $80 per month for adults, $65 per month for children, $200 for families (3+ persons in household)

Counseling Session (1 hour): $130

Our Process

  • Read through the eligibility criteria to determine whether to apply for financial assistance

  • Complete the application for financial assistance. Access this application by clicking APPLY at the top of this page.

  • Our non-profit board will review your application. Expect a response from our team within 2-4 weeks.

  • Choose from our list of approved providers HERE for your health care or counseling services. You will pay your co-pay, Vitality Health Collaborative will pay the remaining financial balance directly to our community partner.

  • Called to bring hope to those who are hurting, to restore what feels broken, and to offer freedom, comfort, and healing where it is most needed. From places of grief and loss, new strength, joy, and dignity are cultivated, rooted deeply and growing toward wholeness.
