Financial Assistance

Vitality Health Collaborative offers a Sliding Fee Discount Program to ensure that individuals and families with limited financial resources have access to mental health and primary care services. Discounts are determined based on gross annual household income and household size.

Household income includes wages, self-employment income, unemployment benefits, disability, child support, and other regular income sources.

Sliding scale eligibility lasts 12 months.

Standard Service Rates

Direct Primary Care Membership: $80 per month (for individuals ages 18-64), $200 for families (2+ persons in household)

Counseling Session (1 hour): $130

  • This sliding scale applies to individuals pursuing financial assistance for DPC services. Your monthly DPC membership fee is determined by your annual gross income:

    Individuals whose annual gross income is between $0-$10,000 would pay a monthly DPC membership fee of $10.

    Annual Gross Income : Monthly Membership Fee

    $0-$10,000: $10

    $10,001-$20,000: $25

    $20,001 – $30,000: $40

    $30,001 – $40,000: $50

    $40,001- $50,000: $65

    Individuals whose gross annual income exceeds $50,000+ would pay full price membership prices.

  • This sliding scale applies to families (2+ persons in a household) pursuing financial assistance for DPC services. Your monthly DPC membership fee is determined by your combined household annual gross income:

    Families whose annual gross income is between $0-$30,000 would pay a monthly DPC membership fee of $10.

    Annual Gross Income : Monthly Membership Fee

    $0-$30,000: $10

    $30,001 – $50,000: $25

    $50,001 – $70,000: $40

    $70,001 – $85,000: $50

    $85,001 – $100,000: $65

    Families whose gross annual income exceeds $100,000+ would pay full price membership prices.

    Individuals whose annual gross income is between $0-$10,000 would pay a counseling session fee of $15.

    $0-$10,000: $15

    $10,001-$20,000: $30

    $20,001 – $30,000: $55

    $30,001 – $40,000: $80

    $40,001- $50,000: $100

    Individuals whose gross annual income exceeds $50,000+ would pay full price counseling fees.

Our non-profit board will review your application. Expect a response from our team within 2-4 weeks

Choose a community partner from our list HERE for your health care or counseling services. You will pay your co-pay directly, Vitality Health Collaborative will pay the remaining financial balance directly to our community partner
